TRANSITIONAL LIVING PROGRAMS

No one makes the transition from childhood to adulthood in
one great leap. It is a process filled with hard lessons and work. For some, the
need to be independent comes very fast, with little experience and few
opportunities. We offer teens and young adults who have been living
in out-of-home care the opportunity to gain actual experience living on their
own while in state custody.
We operate many apartments and serve
young men and women as well as young parents with children. (We do not
assume responsibility for any resident’s dependent children). When a young adult is ready to move into a single apartment,
we help locate one. We
pay rent and other apartment expenses so the young men and women can continue to
focus on successfully completing the program and build a savings account.
Location
Program sites are located in culturally diverse
neighborhoods. In considering a location
for our apartment program, an emphasis is placed on:

Easy access to public transportation

Moderate safety for adolescent/young adult

Location to necessary conveniences

Availability of employment
-
Rental prices that reflect what the teen in the program will be able to
afford when they move into the community on their own.

The per diem daily placement charge pays for the below
services provided to each young adult in the program.

Apartment Rent
(Deposit/Rent)

Start-Up Supplies
Needed (all small appliances, linens, furnishings, cook ware,
utensils, etc)

Apartment Utilities and Phone
(local phone connections only)

Weekly Grocery Allowance
-
Life-Skills Instruction
(learning to use transportation system, laundry, housekeeping,
cooking, and legal rights)
-
Ongoing Counseling
(up to 2 hours per week, includes social skill development and
situational counseling)
-
Referrals to Community Services
(including on site drug and alcohol out patient treatment through
TFI or specialized therapy)
-
Learn
difference of needs and wants (work with the young adult to pay
restitutions, develop savings, enjoy some luxuries based on any
incomes earned)

When a
referral is made, the program staff will interview the young adult and knowledgeable
persons via phone regarding the young adult's ability to live independently.

Young adults, age 17 to 23 years old, in state custody

Demonstrated care of property

Demonstrated ability to follow societal rules

Demonstrated desire to be successful
-
Has shown the ability and willingness to take personal responsibility for
their actions

Demonstrated some stability in past placements

Demonstrated ability and willingness to find and maintain employment

Service Programs
Pathways Transitional
Living Program
The Pathways Program
utilizes scattered site apartments for its transitional living program. The Pathway Program
is located in Kansas City and east Wichita. Program case managers and program facilitators
meet with the Pathways clients weekly to monitor the young adult’s progress toward their goals
and provide on-going transitional living planning.
Teens Reaching
Adult Independent Living (TRAIL)
TRAIL is a transitional
living program operated by Pathway Family Services. TRAIL apartments are located together
in the same apartment building in Topeka. TRAIL provides a live-in on-site independent living
specialist to provide support and monitoring of the young adults in the program. Individual,
group situational, and instructional services are provide on-site.
